We are seeking a compassionate and experienced Social Worker to join our team. The successful candidate will play a vital role in supporting our patients' social and emotional well-being, helping them navigate their recovery journey and achieve their personal goals.
What You’ll be doing
Conduct comprehensive social assessments and develop individualized care plans.
Provide counseling and support to patients and their families.
Assist patients in accessing community resources and services.
Collaborate with a multidisciplinary team to ensure holistic care.
Advocate for patients' rights and needs within the healthcare system.
Maintain accurate and up-to-date case records and documentation.
Participate in discharge planning and follow-up care.
Provide education and training to staff on social care practices.
What You’ll Need
Degree in Social Work (BSW or MSW)
Registration with the Health and Care Professions Council (HCPC).
Proven experience in a healthcare or mental health setting, preferably with complex needs.
Strong assessment and counseling skills.
Excellent communication and interpersonal abilities.
Ability to work independently and as part of a multidisciplinary team.
Compassionate and empathetic approach to patient care.
